The massage chair is provided by Massage Lounge Co for you to experience the benefits of stress reduction and therapeutic massage. The massage chair is designed for your comfort and support in common areas of concern which may include, but are not limited to: the head, neck, shoulders, upper back, mid back, lower back, arms, legs, sides, and band muscles. 

By signing below, you acknowledge that you have scheduled one or more massage chair sessions with Massage Lounge Co. You understand the massage should not be performed under certain medical conditions and consent to the best of your knowledge that you are physically and mentally healthy enough to receive the message. You agree and understand that if you are pregnant, you are required to stay within the recommended setting for the massage chair.

Waiver of Liability

I declare the following: 

1. I am over the age of 18 years old. 

2. I have told Massage Lounge Co if I am pregnant, and I have consulted my medical professional on any risks that I may be subjected to by undergoing these services with Massage Lounge Co. 

3. I do not have any injuries or other medical conditions that would prevent me from receiving a chair massage, nor have I been told by a health provider that I should not receive massage therapy.

4. I have not started any new medications, including antibiotics, in the last 48 hours.

5. I understand that I may be asked questions about my general health and physical condition and that I am obligated to be truthful about my health history.

6. I understand that massage therapy and bodywork are for the purposes of stress reduction, relief from muscular tension or spasm, general relaxation and improvement of circulation and energy flow. It is not a substitute for medical care and qualified medical diagnosis.

7. I understand that a chair massage is therapeutic and not medical services.

8. I will report any discomfort or pain at any level to Massage Lounge Co.

9. By signing this waiver, I hereby for myself, my heirs, administrators, and assignees, waive and release any and all rights and claims for damages that may hereafter accrue to me, or which I have against Massage Lounge Co, for all and any injury, loss, pain and suffering, and damages suffered by me during the course of or in any way connected to this chair massage session.

By booking an online appointment(s) and/ or purchasing a massage, you agree with and understand the proceeding policy and waive liability for you or your minor.
